This is a list of items I put together as a newly 80 Tank. None of these items are found in Heroics. You dont need all of them to get 689 Defence Rateing, if you have the helm enchant from your friendly neighborhood Crusaders.

Wyrmrest Rep items:
Chest
Boot
Cloak
Blacksmithing items:
Gloves
Leg
Bracers
Helm
Belt
Shoulder
Jewelcrafting items: (these are high cost items, hit me up if you are short)
Neck
Ring
Instance items:
Gun'drak quest
The Oculus quest
Halls of Stone quest

There are no tanking 2h weapons, barring a green quest reward from Icecrown. I would highly advise picking up the Blacksmithing made 2h, or see if you can get one of the 2h weapons from H ToC by running as a dps.
